RE:  Haileyville Public Works Authority Water System Upgrades and Projects for System Improvements

To all Water System Members,

We constantly strive to provide the best service and best water quality to all our members. These efforts, as well as system maintenance, require funds to accomplish. These burdens must be borne by you the member, unless another source of funds/grants are found.

We have been working with our Engineers, Infrastructure Solutions Group (ISG), to try and identify other source of funds for these projects. There are funds provided through the Indian Health Service (IHS), however, in order to utilize this source of funding, we need to provide some information to IHS, namely the physical address of Native American Descendants who utilize our system.

If you or any members of your household are of Native American Descent, please write your 911 or physical address on this request and return it to the Haileyville City Hall office at your earliest convenience. We do not need your name or any form of documentation, such as a CDIB card, nor do we need any documentation of tribal affiliation. You do not have to hold any tribal documentation to answer this request, simply whether you or a household member(s) are of Native American Descent and your physical or 911 address.
